What the scorecard looks at

  • Registration status and pathway fit from the facts supplied.
  • Policy currency and control of the policy index.
  • Incident, complaint and risk registers.
  • Worker screening and HR record structure.
  • Incident and complaint handling in practice.
  • Evidence organisation and representative material.

O1 intake

Tell us what is true today.

Use NONE or UNSURE when something does not exist or cannot be verified. Do not create a document just to make the intake look complete. The form mirrors the E3 field set in the delivery specification.

Secure intake: submit the completed form with redacted or de-identified material wherever feasible. The 48-hour clock starts only after Clearharbour confirms the intake is complete and requested materials are readable and accessible.

3. Dates and reason

Optional. Use a secure link or a redacted working copy.

4. Evidence and documentation status

Choose CURRENT, PARTIAL, NONE or UNSURE. “No events yet” is not the same as “no process needed”.

5. Secure material handoff

Use Ctrl/Command-click to select more than one. Select NONE_AVAILABLE if nothing is ready.

If any available category is selected other than NONE_AVAILABLE, provide a file or approved secure URL before the intake can be marked complete.

6. Context and debrief

Before you submit

Do not upload participant names, NDIS numbers, diagnoses, clinical notes or worker personal information. Use redacted or de-identified working copies wherever feasible. If a document does not exist, select NONE; do not create one for this form.

Written debrief

Receive the report, then ask by email.

The scorecard is followed by a written debrief that restates the ratings, evidence limits and priority actions. Email Q&A keeps the decisions and assumptions visible.

Optional live discussion

A live discussion is never required for the scorecard. If one is useful, request it after delivery; availability and scope are confirmed separately.

Regulatory scope note

For the NDIS transition, the local due-diligence canon uses this narrow wording: 1 July 2026 starts the mandatory-registration rollout; new SIL/platform providers need registration, while existing unregistered SIL providers have a transition and must apply by 1 October 2026 to continue.
